Register and Pay. Complete the registration form with patient details and submit payment of $425. This price covers lab testing, mobile phlebotomy, and physician consult. No prior doctor’s order is required

Collection Kit and Mobile Phlebotomy. Religen DX ships a collection kit. Alpha Phlebotomy Group, specialists in autism and special needs phlebotomy, will contact you to schedule a convenient in-home blood draw.
